Why Should You Act?

Hyperglycemia is associated with poor outcomes in hospitalized patients. Hospitalization represents a frequently missed opportunity to diagnose and/or optimize the care of patients with diabetes by providing appropriate management of disease and patient education.

Despite the availability of guidelines and the establishment of evidence-based methods to achieve optimal glycemic control in the inpatient, there are several challenges that exist, including deficient patient adherence to medication, insufficient patient education and lack of coordination of care between inpatient and outpatient providers.

Electronic Quality Improvement Programs (eQUIPS) are web-based online collaborative programs designed to help jumpstart hospital-based quality improvement and patient safety activities.

Glycemic Control (GC) eQUIPS is SHM’s signature online program that focuses on optimizing the care of your inpatients with hypoglycemia, hyperglycemia and diabetes.

It offers providers a performance tracking and benchmarking resource as well as an online community for resource sharing and peer-to-peer interaction.

SHM’s Glycemic Control Mentored Implementation Program 

The goal of this program is to successfully operationalize glycemic control-focused quality interventions at your institution with the added support from leading experts in the field.

When enrolled, your institution will be assigned a mentor site. The program offers a series of conference calls, webinars, e-mails and on-site visits to provide continuous support and guidance from the mentor site. This unique approach allows your institution to utilize their mentor's expertise in a one-to-one professional relationship.
